Please refer to the provided new hire checklist to ensure that all necessary items are completed. We have both a comprehensive new hire checklist and a checklist specifically tailored for seasonal employees.

New Hire Benefits Enrollment

To streamline the onboarding process, new employees are encouraged to complete all necessary paperwork electronically through ADP . However, if electronic submission is not feasible, please ensure that you fill out all required new hire forms provided below.

Once registered in ADP you'll have the opportunity to complete various information online, including personal details, emergency contacts, W-4 forms, direct deposit setup, and benefits enrollment.

For full-time benefit eligible employees interested in participating in LFA's 403(b) Retirement Plan or Health Savings Account Plan, please complete the following forms LFA's 403(b) Retirement plan and Health Savings Account.
